If your child has had repeated urinary tract infections or your doctor suspects a kidney problem, you may have heard the term "vesicoureteral reflux" (VUR). It sounds alarming, but a new study from Saudi Arabia offers reassuring news about how doctors can detect this condition more gently.
What is vesicoureteral reflux, and why does it matter?
Vesicoureteral reflux is when urine flows backward from the bladder into the kidneys instead of flowing out normally. While it sounds serious, many children with mild reflux do perfectly well. However, severe (high-grade) reflux can increase infection risk and potentially damage the kidneys over time, so detecting it early matters.
Traditionally, doctors use a test called VCUG (voiding cystourethrography) to diagnose VUR. This test involves inserting a catheter, injecting contrast dye, and taking X-rays while your child urinates. As you can imagine, it's uncomfortable, exposes children to radiation, and can be distressing for both child and parent.
What did the research find?
Researchers in Saudi Arabia studied 406 children aged 0–14 years who had both an ultrasound and a VCUG. They wanted to know: could ultrasound alone predict which children had serious reflux?
The good news: ultrasound showed several reliable warning signs of high-grade VUR. When doctors saw swelling in the kidneys (hydronephrosis), dilated ureters, thinning of the kidney tissue, scarring, or an abnormally full bladder on ultrasound, these findings were strongly linked to severe reflux.
Most importantly, when ultrasound looked normal, doctors could be 96% confident the child did NOT have high-grade VUR. This "negative predictive value" of 96.1% is the study's most powerful finding for parents: a normal ultrasound is highly reassuring.
What does this mean for Singapore families?
In Singapore and across Asia, healthcare providers are increasingly looking for ways to reduce unnecessary radiation exposure in children—especially important in our region where healthcare standards are high and families expect evidence-based, child-friendly approaches. This study suggests that ultrasound, which is non-invasive, radiation-free, and readily available in most Singapore clinics and hospitals, can be a smart first screening step.
However—and this is crucial—the research also shows ultrasound alone cannot confidently diagnose high-grade VUR. The positive predictive value was only 31.7%, meaning many children with abnormal ultrasound findings won't actually have severe reflux. This is why doctors still need VCUG to confirm the diagnosis, but ultrasound can help identify which children genuinely need it.
For Singapore parents, this means fewer unnecessary invasive tests, less radiation exposure, and more efficient use of healthcare resources—while maintaining diagnostic accuracy.

- Ultrasound is a trustworthy first step. If your child needs kidney screening (perhaps due to recurrent UTIs), an ultrasound is a gentle, effective starting point. A normal ultrasound is genuinely reassuring.
